The page source credits “two [ minimally.livejournal.com ]”, but the layouts link there has been deleted :C Closest I could get is an archived snapshot from 2013, but alas none of the individual posts are archived.
I was able to find the coder (for the record, LJ community → admin’s website link; didn’t call for any deep digging); their main Tumblr is no longer active, but their about page lists sideblogs (one of which updated within the week) and a Twitter handle, so you may have luck contacting them directly for the layout code :o
